Each year, Community Hospice has the distinct honor and privilege to care for our friends and neighbors who turn to us during their time of need. By offering community-based palliative and hospice care, we focus on the medical, physical, spiritual and psychological support needed to assist patients and families. Our goal is to empower those we serve to make decisions that are best for them. Through compassionate quality care, symptom management and education, many of our patients are able to maintain independence and participate in meaningful life experiences.
Our clinical staff continues to provide quality care and support to so many in need, including those diagnosed with COVID-19, throughout our communities. Through regulatory waivers, our staff utilizes advanced technologies to provide tele-visits and connect our patients to their family and caregivers who are unable to be with them. We are so glad to be back at the bedside with our patients and families, but can quickly adapt utilizing the technologies implemented at the onset of the pandemic, should the need arise again. Our dedication and loyalty to our patients and families has never faltered and will remain our focus. We are staffed 24 hours a day, 365 days a year and are just a phone call away.

Palliative Care

Community Care Choices is a community-based palliative care program that focuses on providing compassionate care and relief of symptoms for patients with chronic or serious illnesses. Under the direction of an experienced Medical Director, Community Care Choices provides symptom management, education and support, enhancing comfort and improving quality of life for those with an advanced illness.
Palliative Care may be provided at any time during an illness and delivered while seeking curative and life-prolonging treatments. Our program staff consists of Nurses and Medical Social workers who make visits to patients wherever they call home. Hospice Care
It is often thought that hospice is only about dying and a place one goes when there are no other options; nothing could be further than the truth. Community Hospice provides the highest quality care that focuses on comfort, quality of life, and the wishes of the patient and family. Care is provided wherever the patient calls home, including private residences, assisted living facilities, nursing homes and at the Alexander Cohen Hospice House. In some circumstances, we may also provide hospice services to fragile hospital patients requiring frequent interventions to address symptoms, if those symptoms cannot be managed in the home. Regardless of the setting, our staff apply their expertise to the care of both our patients and families/caregivers. We walk along side them and provide the clinical and emotional support to weather the path ahead. Support is provided by our interdisciplinary team which includes our Medical Director, Nurse Practitioners, Nurses, Attending Physicians, Social Workers, Interfaith Chaplains, Home Health/Hospice Aides, Dietician, Volunteers and Grief Facilitators.

In 2021, we provided hospice care to 1,471 patients and their families. We are truly grateful for the trust and assurance that our healthcare partners place in our staff to provide compassionate and quality care for their most vulnerable patients and their families. Community Hospice Alexander Cohen Hospice House

Our one-of-a-kind inpatient hospice facility located in Hughson, California, includes 16 private inpatient rooms with all the amenities of home for patients and families, and delivers comprehensive 24-hour nursing care. For more than 17 years the Hospice House has provided compassionate and quality care for patients and families in our community. Our dedicated staff consists of both Registered and Licensed Vocational nurses, Medical Social Workers, Chaplains, Dieticians, Home Health/Hospice Aides and Volunteers. Patient care is overseen by a skilled and knowledgeable Medical Director and the Alexander Cohen Hospice House Administrator who is a Nurse Practitioner.

Education

Though COVID-19 still causes some hindrance, we are proud to serve our community by offering a variety of educational opportunities and look forward to a future with fewer limitations. Dedicated to providing education and resources to our staff, patients, families, community residents and healthcare professionals, we offer several annual educational events providing Continuing Education Units (CEUs) for clinicians in the Central Valley.
The Public Health Crisis has continued to help Community Hospice enhance the way we provide educational opportunities for our staff. We are excited to begin implementing in 2022 a new learning management system, that will allow us to increase CEUs and hospice education to ensure we stay in compliance.
In 2021, we were pleased to invite Dr. Joy Berger, owner and CEO of Composing Life Out of Loss to be our keynote speaker for National Nurses Week. Her virtual presentation given over Zoom was, Cultivating Empathy: Head, Heart and Action. Having worked in hospice environments in her past, Dr. Berger offered the ability to relate closely with what our clinicians and other providers have endured throughout the lingering healthcare crisis.
This spring we plan to host inspirational keynote speaker Allison Masarri for her, Heart and Soul of Healthcare presentation. This event will be co-presented by Memorial Medical Center of Sutter Health and Modesto Junior College Foundation. Allison was scheduled to come in May of 2020 and again in 2021, but due to COVID-19 restrictions and a desire to have her present in person we have postponed hosting this event. Please mark your calendars for Monday, May 2, 2022, to experience Allison Massari live at the MJC auditorium.
On October 7, 2021, we hosted Rebuilding Life After Loss, a hybrid event focused on faith, that was both in person and virtual. We were pleased to host over 40 people in our conference room and another 25 on Zoom for our event. Our keynote address was provided by celebrated Author and Grief Specialist, Anne Marie Lockmyer. Attendees were moved to hear the heart wrenching and equally triumphant story of Dr. Vickie Mulvaney Trask. Pastor Brian Hunt took us on a journey of overcoming regret through faith. Therapist Ron Gray provided hands on tools for supporting a survivor of suicide and finally our own Community Hospice Chaplains gave a peek into what their day-to-day world looks like in support of those facing end-of-life journeys. Community Hospice Chaplains each shared from their compassionate hearts and touched the emotions of all in the audience.
Our Community Connections, Your Bridge to Resources series provides valuable educational support and resources for community members, not only for themselves but also for their loved ones, friends and neighbors. In 2021, we provided free monthly virtual seminars via Zoom to our communities throughout the Central Valley and beyond. In addition, this past year we were able to support our partners at Sutter Memorial Medical Center by collaborating to offer our community Caring for the Caregiver, a free virtual educational series for caregivers. Together, we had more than 430 community members join our virtual seminars. Lecture focused on teaching and providing resources for patients and their caregivers during changing times and stresses that we have not encountered before. We are so grateful for the opportunity to partner with Sutter and proud to help our community grow stronger together. Vaccines and Personal Protective Equipment availability allowed us to open the doors to more nursing students this past year. We were pleased to be able to ensure that every student in need was able to rotate through and gain palliative and hospice experience. Students came from California State University Stanislaus, Stockton and Modesto Junior College. Each student was able to complete daily ride a-longs and several students completed a 6-week long preceptorship. We also extended our educational opportunities to a certified nursing assistant, social worker interns and two nurse practitioners who were looking to further their education in end-of-life care.
Our in-house library is open to the public and regularly updated at both our Stanislaus and San Joaquin offices. Our libraries provide a variety of books and resources related to health, aging and grief topics.

Camp Erin® of the Central Valley

Camp Erin of the Central Valley provides a unique opportunity for youth to increase hope, enhance self-esteem and to especially learn that they are not alone. The fun environment of camp is combined with grief activities that give campers a chance to bond with peers, build friendships with others who share similar situations, express their feelings, and honor their loved ones. Led by grief professionals and trained volunteers, camp provides an environment to explore grief feelings, learn coping skills and make new friends. Camp Erin of the Central Valley was created and is supported in part by The Eluna Network.

Due to the public health emergency crisis, our overnight weekend camp experience was cancelled. However, we were able to quickly pivot and were pleased to offer a Day Camp experience for campers who were grieving the death loss of a loved one. Camp was held on August 21, 2021, at our Spyres Way campus in Modesto.
Forty-nine campers joined us for a camp experience that included traditional camp activities as well as grief education and an opportunity for children to honor and memorialize their loved ones.
We are looking forward to holding our traditional Camp Erin of the Central Valley at Foothill Horizons on June 10-12, 2022. To learn more or to apply to be a camper or volunteer visit camperincentralvalley.org.

Grief Support Services

Grieving the death of a loved one can be a painful and lonely experience. Grief is a normal and natural response to loss that can include a whole array of feelings that deserve attention and care. Community Hospice is here to help people walk this path. We provide grief support for children ages six and older, for adolescents and adults.
Grief Support Services are available through multiple programs including:
In 2021, amid loss and constant change, our Counselors and Grief Specialists supported a total of 439 children. Individual and group support was provided to 203 children in 70 area schools including Stanislaus, San Joaquin and Merced counties. Our team works hard to meet as many needs as possible to strengthen our young and vulnerable grievers.
Earlier this year we had the privilege to serve two sisters ages 6 and 13 who had first lost their baby brother by Sudden Infant Death Syndrome (SIDS) and shortly thereafter their father died of COVID-19. The children began receiving individual grief support services at the beginning of the school year and attended our Camp Erin of the Central Valley 2021. They also attended the “Modesto on Ice Camper Reunion” in December. They were participants in our holiday Adopt A Family program as well, and were blessed to have their needs and some of their wants met, with what otherwise, would have been a very scarce Christmas. The children began sessions with many teary moments. As their grief journey progressed they were able to talk about the memoires with their baby brother and father with a smile. 2021 was a difficult year for this family with so much loss and grief. Their mom shared, “I feel they benefitted a lot from the services and have gained healthy coping skills. It was 100% helpful and gave them someone to talk to. The kids loved Camp Erin and had nothing but good things to say. Most importantly they were able to meet other kids that were going through the same experience and were able to re-engage with other campers”.
Grief support was offered to over 1,110 newly grieving adults with 380 individual grief support sessions. Our Crisis Response Teams that respond to sudden, unexpected death circumstances, supported 236 children and 64 adults.
In addition, we provided our annual Coping with the Holidays and Family Tree Decorating Ceremony. Due to the pandemic, both events were offered in a virtual setting and were received with great success. This year’s theme for Coping with the Holidays was Choosing Hope: Finding Joy After Loss, our speaker Karina Castillo is a Licensed Clinical Social Worker with 25 years of experience working in the field of social work, counseling, and hospice bereavement. She encouraged us to lean into the pain of grief and gave us hope that one day there will be more joy than pain. She shared an activity that can help us remember our loved ones by including them in some of our holiday traditions. Our Family Tree Decorating Ceremony was able to honor 56 of our patients and their families. This beautiful ceremony included the lighting of our Christmas tree, and melodious music provided by one of our hospice volunteers. We are grateful for the trust of those we serve.

Hope Counseling

The opportunity to be present for people who have relational and additional mental health needs is a unique privilege. To meet the needs of our community, Hope Counseling was launched in September of 2021. Hope Counseling offers compassionate and high-quality care using evidence-based practices to meet the needs of people with family, mental health and behavioral issues. Therapeutic support is available for children, teens, individuals, couples and families and is facilitated in a nurturing and safe environment that encourages hope and healing. Our highly skilled and dedicated clinicians are trained, experienced and either licensed clinical social workers or credentialed social workers. Each is highly experienced with varying experiences and expertise.

Volunteer Services

Our Volunteers continue to be the heart of Community Hospice. We are filled with appreciation and gratitude for their continued support. This year our Volunteer Department has worked diligently to keep our volunteers engaged and to continue to support our patients and families. In 2021, 502 Volunteers selflessly dedicated 13,774 hours of service!
Our Volunteers have opened their hearts and have shared their talents, providing support to our patients and families as they face life changing journeys. Throughout the year, despite the pandemic, our volunteers have continued to work tirelessly by providing support via telephone, dropping off mementos or sending cards to our hospice patients. Our volunteers at the Alexander Cohen Hospice House have continued to support as they sing and play the ukulele, guitar and saxophone in order to bring comfort and joy to our patients and families. We have also welcomed back our students and have provided them opportunities to work alongside our experienced and compassionate staff as they gain the skills to work in their desired field.

Hospice Compare
Under the Affordable Care Act, Hospice Compare requires all Medicare certified hospice agencies to submit specific data on a regular basis. This data is analyzed and reported publicly, allowing consumers to compare the quality of care provided by Medicare-certified hospice agencies throughout the nation.
Due to the COVID-19 pandemic, The Centers for Medicare and Medicaid (CMS) have temporarily frozen their Hospice Compare data collection. Many care performance waivers have been established to assist health care providers during this challenging time. Community Hospice has not elected to use many of these waivers in 2021, such as Hospice Aid (HHA) or Continued Education Unit (CEU) requirement. CHI has elected to continue the HHA/CEU education provision program ensuring that our Hospice Aids have the most up-to-date patient care information available to them. Hospices have also been granted a waiver to change the Registered Nurse (RN) visit frequency from every 14 days to every 21 days during the COVID-19 pandemic. CHI has elected to keep and maintain our RN patient visit schedule of a visit every 14 days. At CHI we are doing our best to function according to our pre-pandemic high standards. Community Care Choices Community-Based Palliative Care Program collaborates with the Palliative Care Quality Collaborative (PCQC). PCQC aims to improve comparison reports, align with national standards, inform, research and increase quality improvement, and explore Electronic Medical Record (EMR) integration. This organization allows us to submit our patient data and compare our palliative care program to similar programs. We provided care for 183 patients and made 1,915 Registered Nurse and/or master’s in social work patient care visits by the Community Care Choices team in 2021.
